Please read before using Vesper

Disclaimer & user agreement

Vesper is a documentation aid used at your own risk. You — not Vesper — are responsible for making sure no patient identifiers (PHI) are copied or pasted into your charting program and for the accuracy of everything you chart.

Use at your own risk

Vesper is provided "as is" as a convenience tool to help draft clinical documentation. Use of Vesper is entirely at the risk of the person using it. Vesper, its developers and operators make no warranties of any kind, express or implied, including accuracy, completeness, fitness for a particular purpose or regulatory compliance.

You are responsible for protecting patient information

Vesper attempts to remove patient identifiers automatically, but de-identification is not guaranteed and may miss names, dates, places, numbers or other protected health information (PHI).

It is solely your responsibility — not Vesper’s — to avoid dictating PHI where possible, to review every scrubbed and formatted note, and to make sure no PHI is transferred when you copy and paste into your charting program (EMR/EHR) or anywhere else.

You are responsible for following HIPAA, your state’s privacy laws and your employer’s or agency’s policies on approved software, devices and the handling of patient information.

You are responsible for what you chart

Vesper may reword, reorganize, add compliance framing or default wording, or make mistakes. Formatted notes are drafts only. You must read, correct and confirm every word before it becomes part of a medical record, and you remain fully responsible for the content, accuracy and completeness of your documentation.

Vesper does not provide medical, legal, billing or regulatory advice, and its "CMS documentation check" and wording suggestions do not guarantee that a note meets Medicare, CMS, payer, survey or agency requirements.

Limitation of liability

To the fullest extent permitted by law, Vesper and its developers and operators are not liable for any disclosure of patient information, documentation error, claim denial, audit finding, survey deficiency, employment action, or any other loss or damage arising from your use of Vesper or of any text it produces.

Your agreement

By selecting "I agree" or continuing to use Vesper, you confirm that you have read and understand this disclaimer, that you accept full responsibility for keeping PHI out of anything you copy or paste and for everything you chart, and that you use Vesper at your own risk.
